I don't think this is right at all; those are not the only two options and not what people mean. Many do mean control costs.

right. charging $2000+ for a head CT, for instance, when the same test costs 1/8th of that in Europe or Japan, is absurd.

It is nuts. And no predictability. You get a bill. Gloves, $800. Oops, you have insurance. Now they are 80 cents.
