      1/6 Important new study from Japan finds that with Omicron, infectious viral loads peak 3-6 days after symptom onset/diagnosis. So many people are ending isolation & returning to work & school at peak infectiousness.       There are two issues. Are rapid tests useful for ending/prolonging isolation? I think the answer is yes. Second issue is: if you don’t have enough of them, what are you gonna do as a coping strategy? That is also worth looking into, and ideally based on data like this.

          Nothing is ideal without tests, but if it is bimodal, I’d be curious about spring the boosted/vaxxed without symptoms on day three and keep anyone with symptoms until two consequent symptom-free days, on or later than day five as one potential trade-off. (Wearing good masks).

          Also, are folks ignoring the additional conditions for ending symptomatic isolation ie fever-free for 24 hrs and improving symptoms? I’m not sure how much of difference they make but different than headline 5 days isolation.

          I don’t see much evidence of fever being a reliable indicator here tbh. That’s not what I’m hearing from people testing positive for a long time, so not sure. Would like to see if there is any data supporting it?

          Or just making a billion more rapid tests that are easily mailable? Don’t understand why we aren’t doing this. Abbott’s BinaxNOWs are flat cardboard, no dumb app, and my favorite for this reason.

          Rapid tests can sample the correct paths of contagion. Someone should double test — simple nasal swab & throat swab and update CDC/FDA data for EUA for each product. Testing is hard, needs testing to purpose. Ok to go to work. Ok to return. Is the work place safe (enough?).

          Two comments. One, I did have tests and "tested my way out", day 13. Tests on days 4 and 7 lit up immediately. ...

          Gottleib on TV today mentioned stat that "only" 10-13% are still positive on day 6, so not a huge driver of new infections. Suggested what you do as an individual after day 5 should depend on your circumstances vis a vis contact with high risk folk.

          3rd issue is: How many absent staff can the US infrastructure (healthcare, education, transportation of both freight and humans, sanitation, food production) take without causing chain reactions that lead to long term damage?
