      The decision not to use rapid tests here (but then say you're buying 500M of them, ramping up test-to-stay in schools) is mind-bogglingly incoherent.

      I think the reality is that we don't have the tests. That 500 million is a target starting in January. Won't ramp up immediately. An alternative is to triage test capacity, direct it to health-care and other critical settings. Not happening either. Not enough tests to test with.

      Similarly, capacity-saving triage to end isolation in five days without tests, use only source control via masks should talk about masks with better fit-and-filter, maybe differentiate between vaccinated/prior-infected, etc. But you can't do it overnight, and we didn't till now.

      The interchange in another thread between @mugecevik and @NathanGrubaugh is key here: we haven't seen the empirical data or modeling on which this is based. We don't know the trade-offs that are being made.

      Rapid tests may not be predictive at the individual level with absolute certainty, but I find it hard to believe they are not probabilistically predictive at population level—as a public health tool. Ceteris paribus, better containment with test-to-stay than "X-day and done", no?

      The aim of isolation should be to ensure more ppl are isolating at the highest risk period - first 5d. Exit antigen testing could be confusing bc +ve test in vaccinated individuals later in disease ≠ highly infectious, as VL will be lower at the tail end.       Also don’t necessarily agree w/ comments against CDC recommendation. 5 days of isolation is reasonable based on data, which would capture the majority (>90%) of transmission events & make isolation practical.       What do you think of test-to-stay for schools? Also the US rule doesn’t distinguish between vaccinated or unvaccinated.

          I think that's aspirational. The logistical nightmare of literally millions of tests a day, in the near future, doesn't seem possible.

          Test to stay failed in my small town. They ran out of tests during an outbreak last fall, and fell back to exposure/isolation guidance. I imagine it would be worse than that everywhere now
