(To be clear: this is a thorny and universal problem in all fields that require extra levels of safety because of potentially catastrophic consequences. Authoritarian settings may well be even worse, but nobody is immune to this).

Could be cross-contamination. They are looking into it. That said, one has to report bites as well, as per the rules—can't assume something isn't a path since that's how you'll miss it if it is. Classic ascertainment bias.
