Everyone saying that J&J “should have” made it a 2-shot vaccine forget that it was designed in part to help vaccinate the entire world. J&J or AZ prime followed by mRNA is a great strategy to control the pandemic in LMICs.

We used AZ prime w mRNA second dose spread out 8 to 16 weeks in Canada this year due to supply issues. It has worked great. We also mixed mRNA. Glad to see the USA FDA accept it.

Something nobody has looked at is to prime with Pfizer or Moderna & boost with AZ. I would love to hear some expert opinions on this: possible benefits include more rapid distribution due to good supply, & maybe good VE due to heterologous effects. But evidence needed of course.

There have been a few studies, e.g. https://www.thelancet.com/journals/lancet/article/PIIS0140-6736(21)01694-9/fulltext … That found AZ@1/AZ@2 < Pfizer/AZ < AZ/Pfizer < Pfizer/Pfizer in test tubes. You really wanted to have Pfizer second, and having it for both was even better.
