Fog of Pandemic: We have too little systematic data and too few clinical trials. In my new piece, I recount the story of the dirt cheap drug that may have quietly saved millions of lives this pandemic—and would have been ignored but for high-quality data. https://www.nytimes.com/2021/08/27/opinion/covid-data-vaccines.html …pic.twitter.com/QO3IuV0Csa

In the US, we don't have reliable data on vaccination status of hospitalized, or even children and COVID hospitalization (despite its rarity). We can't sort through waning efficacy, variants and boosters—people are trying to reverse engineer boxplots in screenshots from Israel.

Also, I want to highlight this remarkable thread from @michaelmina_lab from November of 2020 which pretty much tells us where might be in August of 2021. As he keeps saying: this is a textbook virus, and we're not fully mobilizing our scientific knowledge. https://twitter.com/michaelmina_lab/status/1328355643300917249 …pic.twitter.com/rQE3ioq4hU

Also this (read the thread: not an argument against clinical trials). There's a remarkable but hard-to-turn-into-bullet-points process between existing knowledge (especially of mechanisms), data (systematic or not) and policy (judgment/trade-offs/timing).https://twitter.com/michaelzlin/status/1430739705255981063 …

Exactly. Empirical data-gathering is not the entirety of science. In fact it is just a sliver. Our accumulated knowledge of molecular and physical mechanisms (of infection, immunity, dispersion, mutation) predicted everything that has happened. Clinical trial results did not. https://twitter.com/michaelmina_lab/status/1430732978418761728 …

Which ties to this paper on airborne transmission in Science just out. (See thread below). I think the key question is: how can we learn from all this and what can we do to improve that interaction between prior knowledge, new data and timely policy?https://twitter.com/zeynep/status/1431248145497927687 …

Just out in @ScienceMagazine: A review of airborne transmission of respiratory viruses. We were overdue for a paradigm shift in our understanding of transmission mechanisms of all respiratory viruses. Yes. It may take time to build the proper data infrastructure we need, but we should do whatever we need to do in the meantime—last year the remarkable @COVID19Tracking showed what was possible with leadership, initiative and, yes, blood, sweat and tears.https://twitter.com/ScottGottliebMD/status/1431249211308265473 …

Maybe we need to bring back the Covid Tracking Project to provide actionable, real-time information, while the CDC is finishing building out its collection capabilities and mission set. https://www.nytimes.com/2021/08/27/opinion/covid-data-vaccines.html …

Once again the systematic that or we need comes from the United Kingdom. And if it holds up, it explains so much of the current crisis. As suspected, and suggested by prior data, Delta may well be more severe too—read the thread, only for the unvaccinated though.https://twitter.com/kallmemeg/status/1431392586946813963 …

This study found two-fold increased risk of hospitalisation for Delta variant It suggests Delta is not only more infectious, but also potentially more virulent. The good news is that immunity through vaccines seems to mitigate any additional harms inherent to the virus

It seems to require a combination of various scientific disciplines; medical, statistical, sociological, psychological and perhaps even political, to fully understand why the virus has spread the way it has in this country. Complex virus meets a complex society at a complex time.

Yes and no. The thing is that the virus is exactly what we thought it was and the reasonable assumptions made were reasonable and we should have promoted them as such instead of falling back on how they might be unreasonable.
