Thoughtful thread on advisability of mRNA booster after J&J vaccine as Delta invariably increases here. Already boosters are being done on a case-by-case basis, and its not hard to understand why. Too soon for a policy statement? Earlier I proposed a simple public health philosophy: "identify the worst-case scenario using the most up-to-date science and recommend easy things that can mitigate that" No claim to originality; would hope it's the default PH approach actually Let's apply it now to J&J vax... https://twitter.com/michaelzlin/status/1406326680948334596 …

Great thread, thanks for highlighting. Still do not understand why it is almost verboten to discuss the issue of boosters in people who have received J&J vaccine and provide guidance backed by the available evidence on what to do with these individuals.

Not too soon for a policy statement. I am getting this question more frequently from individuals who got the J&J vaccine or their providers. Clinicians would feel more confident in the advice they give if this is backed by a policy statement grounded win science from PH agencies.

I’m struggling a little to understand this one. Why recommend re-vaccinating 10 mill+ people when there is a global scarcity of vaccine and J&J protects severe disease against variants? If it’s transmission risk why not use those resources for unvaccinated pockets in the South?

Even a mild case of Covid has been shown to have significant long term health consequences. If there is a way to increase people’s protection it should be done.

Vaccinated breakthrough is not the same threat as novel exposure esp. as an adult, even if the cases are both "mild". Personally, I'm not as concerned about that for myself, for ex, unless we see data to the contrary. But once global supply is met, such questions may occur, too.

That’s good to know… I still remain very worried about my level of protection against the Delta variant. Thank you for your response and your work

You're very welcome. FWIW, If I had the J&J, my own risk from Delta wouldn't be a personal worry for me at this time. Population-level risks are different questions at times than personal risks.

Vaccinated people are still getting long covid, so that’s very concerning to me https://www.nytimes.com/2021/05/25/health/cdc-coronavirus-infections-vaccine.html …pic.twitter.com/Up6UO3fLbo

Ms. Cohn wasn’t hospitalized, but she experienced body aches, chills and digestive problems for about two weeks. She was left with fatigue, headaches, brain fog and vertigo so severe and sudden that she crashed her car into the garage one day.
She is still not back to her daily three-mile runs with her dog because of shortness of breath. “I’m young, 43, healthy, with no pre-existing conditions, but you often find me now on the couch resting,” Ms. Cohn said. “Don’t people want to know about this?” she asked. “Where do people like me go? What happens next? The practitioners in my life have been shocked and are trying to figure out how to move forward, but there are so many questions. And if no one is studying this, there won’t be answers.”
