The situation in India is dire and all variants associated with a surge need attention. But this man is 88 years-old, diabetic, two prior heart surgeries, and still had only mild symptoms (mild fever was reported) and is hospitalized out of caution likely because he is former PM.https://twitter.com/GabrielScally/status/1384452978086596616 …

The surge in India looks terrifying and reports are horrific. The hospital system may collapse, and India needs immediate support in all ways we can. This is unchecked exponential growth. The potential variant contribution (increased transmissibility?) doesn't change the basics.pic.twitter.com/UehtCPAIMs

There is already encouraging reports about vaccine efficacy out of India—breakthroughs appearing to be extremely rare despite dominance of variants. But that doesn't lessen the reality and the severity of their crisis. Exponential, unchecked growth is a crisis. ht

This doesn't look like trial data (unlikely that crores were followed up in a trial), and given the known undertesting in India, & underreporting it's very likely that these are underestimates. We definitely need to see the control group.

I'd also like more data. But US vaccine breakthroughs are also rarer than even my best hopes tbh. We'll see. That said, India's current, immediate and horrifying crisis does not seem to center around vaccine breakthroughs, and seems to deserve action regardless.pic.twitter.com/IKmA2VUShe

I think context is important here - there are many issues with the way covaxin was approved & rolled out- phase 3 data were never published prior to approval (still not published)- this led to lots of vaccine hesitancy - even among HCWs. There were also many issues with the trial

It's clear that scores of crores of people have not been followed up- which brings up the question as to how this was assessed. And why no control group data have been released. It's worth following commentary from Indian PH experts- there are real concerns about ICMR reporting.

Sure. I'd still say vaccine breakthrough (especially as in the original tweet I highlighted: 88 year old man with health problems has mild fever) seems to be.. well, not a good focus for us given how terrible things are due to exponential growth.
