      Well, didn't forget about TB, but the model is a cautionary one. I did interviews with about 50 people in three countries several years ago, where HIV work was progressing rather well, but wanted to know why TB, maternal health was lagging behind...1/

      I'll never forget several interviews in Brazil where people told me that TB "was" an issue for Brazilians before the advent of drug treatment, but once you could buy a cure, it became a disease of the poor, the favelas, could be ignored by the middle-class and the rich. 2/

      Not saying these interviews were representative, but it's not surprising that once you've got yours (TB therapy, a COVID19 vaccine) your interest in the good of humanity drops precipitously. end/

      That’s exactly why it’s a perfect model here. So many people think once they’re vaccinated they don’t have to care about others getting COVID19 anymore. And support for policies drops off a cliff. Variants are one of the few things that still risk their safety.

      Vaccinated people may well feel protected enough (in ways that is not completely unfounded) for now & the variants are sweeping through the world now. If we don't succeed in the moral urgency argument, it's plausible for natural immunity to, tragically, sweep through billions.

          It's plausible "but variants" works on policymakers without having the intended effect on broader public (who agreeably may not matter). I also won't be surprised if some portion vaccinated people quickly book, say, vacations elsewhere without feeling that much at risk.

          I do think foreign policy/aid issues have a different political opportunity structure than domestic, where pub opinion drives less. overton window open now, but I think you're entirely right that it will easily close. Which is why action now on vaccine equity matters

          The moral urgency argument hasn’t worked well for HIV or TB - the most relevant models here. The argument that preventing variants abroad means fewer boosters, fewer scares and fewer costs in the US is more powerful. It’s an ROI argument not abt “vague grave threat”.
