Important thread below on immunologic basis for skepticism of 0% protection by prior natural infection on reinfection with B1.351 suggested by the small novavax study in S Africa So why else might they have found identical cases by sero+/-? Speculation before seeing full data:https://twitter.com/profshanecrotty/status/1355552010158764036 …

4/ The thornier information was: Infections with the South Africa variant (SA) ("attack rate") were similar in SA between seronegative subjects and already seropositive subjects. https://twitter.com/kakape/status/1354922533997473798?s=20 … pic.twitter.com/tlFWTgyf4b

We have long been aware of intermittent viral RNA shedding long after clinical recovery. Here's one study suggesting median of 23 days pcr + in mild cases and 34 in severe. The bane of clinicians as ppl present for unrelated care months later and still + https://wwwnc.cdc.gov/eid/article/26/8/20-1097_article …pic.twitter.com/0FhWGbUVMI

If I'm reading the novavax presentation correctly, they found 152 total cases, 44 after both doses among seronegatives (primary analysis), of which 90+% tested b1.351. There were 53 cases among 1324 seropositive, which they are calling reinfections.pic.twitter.com/69vUxE9q3N

For these 53 cases among 1324 seropositive, they do not yet report case criteria distribution, severity, PCR cycle thresholds, and (crucially) genetic sequencing. All of these will help to determine whether these were true reinfections, or persistently positive PCR.pic.twitter.com/WkBZME8odD

As a consequence, comparing the raw case rates is likely to bias the estimate in the seropositive group upwards to some extent. CDC provides some guidance on how to investigate suspected reinfections, which to date have been identified only rarelyhttps://www.cdc.gov/coronavirus/2019-ncov/php/reinfection.html …

There may also be something of a behavioral bias (those who were seropos at baseline may be those who are less likely to be able to / choose to avoid virus than those who were seroneg?). But it's weird, & I agree not consistent w/ a lot else we've seen re protection from reinfxn

This is very very important and may explain the similar attack rates in the seropositives and seronegatives in the placebo arm of the Novavax trial in South Africa! https://twitter.com/BNITM_de/status/1369667816274681869 …

I'm remembering a mention of "less severe" somewhere but can't find it. I hope they have sequencing and cycle data in the full EUA.

Yes, full sequencing and ct will be critical info. Haven’t seen the less severe mention. Also in EUA need to see this difference after adjustment for baseline characteristics associated with exposure risk.
