The efficacy endpoint for these trials has been "any symptomatic disease"—even sniffles, headache, fever. That's what the 95% refers to, not "less severe hospitalization & death" which, in the trials (all the trials) have been reduced by 100%. Transmission, is another thing.

Between Moderna and Pfizer Phase III trials, there were 39 severe cases in the placebo group, and 1 case in the vaccine group. So, 97.4% efficacy against severe illness, with a fairly small n?pic.twitter.com/cCIvn3s5rW

Yeah, though the Pfizer severe case is a woman who had 93% SpO2, no hospitalization, no medical care needed and would not even qualify if one percent higher. Fair enough though if you count that single case but that's what it was.
