When (on 1/3) @ashishkjha & I proposed delaying 2nd dose https://tinyurl.com/y7c6v9hz , pushback was strong. In past wk, feels like consensus has shifted this way, driven by race against variants. On @MeetThePress @mtosterholm says we need to "call an audible."https://tinyurl.com/y5q2bfkl

My area is scheduling Pfizer boosters three weeks into the future but is completely out of vaccines to give the first dose to many of the elderly. I know CDC/WHO both said six weeks okay but that's not the practice. About half the elderly remain unvaccinated in my state.

The only other vaccine I know that even allows (let alone recommends) a three-week interval is the Japanese Encephalitis. But here, in many cases, three weeks is being treated almost as the maximum delay allowed, rather scheduling for *after* three weeks but not too late.

The worst part is that after all this brouhaha, there appear to be zero resources to make sure the elderly do show up for their scheduled boosters! The vaccine centers are doing heroic work, but unlike health-care workers, the elderly need support to make it to the appointment.
