      So as the co-author of the piece criticized there as too cautious, I have a question that a model may help answer. Some of the elderly who may need the booster most (phase I/II results) may be least likely to get it—based on observations in a vaccine clinic I'm helping out with.

      I wish we started a trial back in December but here we are. Q: I wish there were models for starting with one dose but then at some point moved to boosters (for elderly going down by age) while expanding one shot for younger groups: varying correlates of protection assumptions.

      The reason I say this is now we see places like UK that just seems to have moved to all one-shot strategy (no trial) while here in the US we are doing two-doses for Pfizer on three-weeks to booster strategy when WHO/CDC both say six weeks okay but not differentiating by risk/age.

      In the meantime, again based on my own observations in vaccine clinics, we're going to have de facto single shot for many elderly because our follow-up strategy isn't there. I'd rather we put more resources to that *explicitly* while pondering delayed boosters as in this piece.

      So, lacking trial data (which is always my preference) I'm asking for models *cough* from people who've made great ones on prioritization before to help guide this discussion. :-D

          I think it will lead to #VaccineHesitancy if people can't be sure, upon being offered first shot, that a timely second shot will be there for them. I for one would rather wait till they're sure of the second shot. Or until a one-shot vaccine is available. #CovidVaccine

          1/2 Did some work on this. In essence - gain in protection by those getting jab 1 outweighs that lost by those losing out on jab 2 but if clinical illness means dying at usual age-specific rates, the death rates overpower the protection gains at about 21 yrs age difference.

          2/2 Protection against severe disease and death is higher than protection against clinical disease - hard to quantify given so few severe cases, but we know it’s so much higher that the apparent reversal based on clinical illness won’t occur for severe illness.
