And we (Michael and I) are advocating for going ahead with a two dose regimen while *immediately* going ahead with a trial—should have started yesterday. Which is the exact point this started from. I'm answering the "weaponization" charge separately.

We may be talking past each other. I am aware of what you an MM are advocating for. I think you two are 100% correct that such a trial is essential.

I do *not* claim that advocating for this publicly is the wrong way to go. I am asking is whether it might be the wrong way to go. There are reasons not to advocate for this publicly. It would be great to discuss those reasons, admit that they have some merit when they do...

Just for clarity. One concern is that this might cause people—who otherwise wouldn't—to skip the booster? Any other? Mistrust in the process? Genuinely asking.

Concern 1: It is very difficult to roll out a reliable two dose vaccine regimen. People will find reasons to skip the second dose, lose paper work, etc. Many do not have regular doctors/PCPs/etc.

Concern #1 continued: If a popular view emerges that people only need 1 dose and that turns out to be wrong (as it might), then we have a big problem. We all for now need to feel all the urgency in the world around the 2 dose regimen.

Concern #2: The right will weaponize the public 1 dose trial rhetoric to further sow doubt about whether public health officials can be trusted. How will this will manifest when people on the right reason about medical care? Experts on communication (you?) should weigh in on it.

Concern #3: Bad actors with power will take the energetic, public proposals for a single dose trial and use it as a justification for giving only 1 dose to poor people and people of color. They will say, “That’s enough for you! We need to save the double doses for the doctors!”

You might think these are all bunk. That is fine. But they are concerns that exist out there. I am so happy to be shown that the balance of reasons supports your approach. But reasonable people who aren’t experts can disagree with you, and can hold the views I’ve articulated.

No, the opposite. I appreciate the rundown. I'll write a longer reply; this is too much substance for Twitter. Probably a newsletter post. Feel free to add any. :-D I think about this stuff all the time, is the truth of it.

(Or if you know anyone—you?—who'd want to write a (paid) guest post for my newsletter on all this, and I can respond longer there). The truth is, all those are risks, and will always be, but I think they cannot be avoided by avoiding the topic.

This gets to the heart of something important, that the public sphere is not one where the gatekeepers control the gates anymore, and for better or worse, it's reality & we have to figure out how to deal with this effectively and in the public interest (something I believe in).
