      Its totally fine to recommend masks & to note strengths & limits w data But let's not go overboard and claim a cluster RCT was impossible.       There are many valid methods to get at these questions. How do you do around my study when you can’t enroll the target beneficiaries in the trial? I don’t think people are fully thinking through the methodological issue at hand here.

          You would need to do community level randomization with incidence as the outcome — like test and treat studies for HIV in Sun Saharan Africa, which gets at the treatment as prevention paradigm. They are difficult trials but can be done.

          The intervention would not be mask / no mask but rather free masks on every corner, mask mandates indoors+/- outdoors, messaging campaigns

          Likely interesting data available on eg meat plants: After the famous outbreak in Germany where a large % of workers were infected despite wearing (single layer) masks, higher quality masks were recommended. I assume many plants made changes. Interesting comparisons possible.

          Comparisons before/after, and betw plants where higher quality masks were used & where they weren't. This would also capture protective effect via source control. Difficulty is that there likely were also improvements made re other precautions, so difficult to isolate mask effect

          A study like this one *could* test protection for others, even if this one doesn't. For each study participant infected, one could trace contacts and ask about positive tests. To the extent that typical mask behavior protects others there should be a measurable effect.

          Avoid the bias by having people prelist frequent out-of-household contacts at enrollment. Maximize power by only enrolling those who prelist many such contacts.
