No, you don't need to forbid the control arm from using cloth masks 👇 In March, many advocated masks were harmful. One group could be advised to wear masks, and the other advised not to. That mirrors the *actual public health interventions* deployedpic.twitter.com/a3O81UB9Va

This is a better point 👇, and the power calc for a cluster RCT (which is horrendous) is a better point, but this is also ironically a limitation of deploying the intervention without RCT.... allow me to explainpic.twitter.com/SOQHR09usj

...there will always be some people reluctant to embrace the intervention, and these folks may also be more skeptical of sars-cov-2, they may be more likely to congregate in large groups.... So deploying the intervention w/o trial faces this real world limit as well.

Ummm, we shall see when the Danish study is published... as that is a 6k person RCT of giving a box of masks vs not in this pandemic 👇pic.twitter.com/HJQmLyCX5M

Finally, without a cluster RCT no one knows the effect size. Does it lower the risk of acquisition by 1% .1% or .01%? That might help inform how much folks should beat on the drum about this issue on twitter But here is the real truth...

The cement has hardened. The toothpaste is out of the tube. Folks have already decided their individual disposition & you can mandate whatever you wish, but how will you enforce & police it? Will that make it better?

Instead we should have taken @mgtmccartney advise in March, and done a cluster RCT And how about for places not running the trial, honest messaging, such as....https://www.bmj.com/content/370/bmj.m3473 …

March: "Look, we totally said that cloth masks would increase risk of transmission last week. We *&^*( that up. Now, we think they are worth a shot. Do we know how well the work, in this politically divided world, with this virus... not really. We don't...

But let's just pitch in and do this so we can all get back faster to doing what we want to. And, look at me, I will set an example and start."

Bottom line: You don't need RCTs for every decision you make in a pandemic, but let's not go overboard and overstate the claims they cannot be done. Turns out every one who likes anything says RCTs can't be done for their pet intervention See also... http://cmajopen.ca/content/6/1/E31.full …

Danish study is *not* about source-control, which is the point of the oped. How do you do a randomized study when the beneficiary is not enrolled in the trial? Or even clear who they are.

That’s a cardinal feature of a cluster RCT design. To look at system or community-level benefits, unlike subject level outcomes in a traditional drug intervention RCT.
