Ignore opsec for a moment. If he's discussing patient details, locations and dating material; that's a big ethics red flag we can't simply handwave away with pseudonymity.
Denying what? Moving the goalposts doesn't make me trust the rest of the argument. I'm conceding there may be a case; people who think so should make the case. I don't need to read or defend every last pseudonymous blog to defend pseudonyms!

defending pseudonyms doesn't require apologetics for scientific racism, and makes your case look much more like special pleading for Slate Star Codex than application of a general principle
