The mechanisms are quite different. For example, even a light scarf might be useful for 1 because it blocks or deflects the air stream, even if it is useless for 2. Or you have the example of many N95 masks with an exhalation vent that makes them unsuitable for 1, but great for 2

Plus, hospitals do aerosol-generating procedures and are sick people all day. (Not surprised that they shouldn't be using surgical masks unless they have to, which is where we are.)
