Here's an important discussion that people are missing in this polarized environment. Not only can we not remain in full lock-down forever because of its human costs, we should not. Good luck discussing that though. (Work by @mlipsitch/@yhgrad and others). https://www.bostonglobe.com/2020/04/10/opinion/its-possible-flatten-curve-too-long/ …pic.twitter.com/eJ3qGpwQz2

If I’ve understood the article correctly, it says that the only strategy we have, absent a vaccine, is herd immunity. But what about #TestTraceIsolate? As per South Korea? If we isolate clusters & trace contacts, offers a third path.

these two aren't mutually exclusive, though. I'd assume Lipstitch agrees with #TestTraceIsolate.

Of course, so does every sane epidemiologists. I just don't understand how people think top scientists are all idiots that must be pointed out the simplest stuff in their own discipline.

Oh absolutely! No doubt that Lipsitch knows this stuff inside out (as do you!). I was genuinely asking how you think the article - which comes out so strongly for a herd immunity approach - fits with what Sridhar has been saying. Real disagreement within discipline re: approaches

So, that's a great discussion because the positions aren't that far apart. But she's counting on a vaccine in a time-frame that may or may not be realistic. Nobody knows, honestly. Her soft-lock down and suppress-release aren't that far apart.

<nods> The timing of a vaccine is defo a crucial part of it. From what I understand, she's also advocating for more time to *understand* the vaccine, and its long-term implications, before we expose large % of pop to it. Which would be a necessary (& risky) part of herd immunity.

There are no good answers. Lockdown (& increased mortality from same) is also awful. And: I appreciated her points in last 2 paras of this: https://www.theguardian.com/commentisfree/2020/apr/22/flattening-curve-new-zealand-coronavirus?CMP=share_btn_tw … i.e. not just playing for time for vaccine, but for more info on virus generally re: who's vulnerable & why.

Perhaps, for US, with where you are, under realistic best case political scenarios. But S. Korea and Taiwan use suppression with semi-normal non-lockdown rules. Australia, NZ, and Vietnam aim for something similar. We would never tend to herd immunity as %age exposed is so low.

Just getting to where I am stuck not trying to explain to you 🙂 To me it appears a clear difference in position. That unless we have a vaccine in quick time, AU/NZ must switch plan to “infect more people” atm even low risk groups have very low % exposed.https://mobile.twitter.com/GaetanBurgio/status/1253640874216394753 …
