Gender isn't entirely based in biology, it's primary sociology. Sex has a base in biology and even within that there are many more possibilities than male and female alone. There's likelihood a cis woman may be born with xome abnormalities and be XY for example.
Actually @misErydactyl is correct. A crazy doctor who experimented on two innocent young men after one of whom's generals was destroyed during the mutilation ritual known as circumcision. They killed themselves as the result of the mental trauma he put them through.

I was not referring to that. I was in fact referring to what is known as an intersex condition. One of which where women can be born with XY chromosomes is AIS. Many women don't even know they have it and are still fully fertile and able to get pregnant.
