It's complicated. Options depend on salary and employment status. For min bureaucracy the "Gesetzliche" is best. They pay the bills, you don't even see them. You can also get that voluntarily as freelancer or with high income. Downside: sometimes difficult to get appointments.

With private insurance, it is easy to get appointments. You get sent the bills, pay them, and get reimbursed by the insurance. There is no difference in the quality of care. Some Doctors try to sell you unnecessary treatments if you have private, because it is lucrative.

Everybody recommends Tk but from personal experience, that's because most people never try to claim anything
I've been pretty happy with barmer (as someone who _actually_ needs health insurance to cover stuff) -
I’m a TK member and claimed a few things (vaccinations for travel, expenses that I had to pay in cash, but that should be covered, smallish stuff) Never any issues. Kinderkrankengeld is a mess, though, if you’re not employed. The entire system doesn’t work well for self-employed.

I've been using TK since moving here. I have no clue if it's the worst or best. But I do know that I still never go to the doctor here after several rather annoying visits and being refused appointments for arbitrary reasons.
