I've been futzing around with a better jira but haven't done much, but ideally issues would be visible and separate design bugs or features. You wouldn't use a spatula to cut a potato so why would you use issues to design software?

Namely having a good collection of tools about how one is and wants to shape code is integral to the dev experience but no one does it right

GitHub issues is too simple and feels like an afterthought, but JIRA is an Eldritch horror and while "it works" it's complexity is hit with bad UX that's so unintuitive it feels like an alien with an idea of what a human wants did it.

Hard agree. Spent all of Thu/Fri talking to GitHub engineers about literally this. Good news: some of them care! Bad news: nothing is likely to change in the next year+.
