There is tremendous value in code school grads. Saying you canonically do not hire from code schools makes your company sound stupid.

One problem is people aren't equipped or willing to to do mentorship, so they shoot for ppl they think won't need to be mentored.

a. Everyone would benefit from a good mentorship program. b. This is making a lot of great engineers "unqualified" for no good reason.

It goes without saying that this affects underrepresented groups more, since by definition to increase representation you need beginners.
