There is an overwhelming perception that spending hours investigating a bug is reasonable, but spending 30 minutes filing a bug is not. Why?
Version seems somewhat ok but I admit to often, as an issue filer, feeling flummoxed when asked to fill that in.

The template should tell you exactly how to find it.

Templates should also start with a free form way to describe what went wrong and end with minimal "template"y stuff.

This structure helps me learn about expectations. I found that often tool works correctly but users have different learned expectations
