Has anyone ever even *tried* defending the common practice among media outlets of running titles without getting authors' approval first?

Titles and subtitles. Editors often slap their own on without running them by authors.

An interesting. I was a student editor and I can explain at least WHY it happens in print. Not sure about web.

The constraints on print headlines are pretty rough and deadlines are tight. Headline writers are skilled at this particular task.

I understand that deadlines are tight, but it really doesn't take long at all. Include it in your edit suggestions!

I've done a ton of editing myself and would never dream of running a title I hadn't ok'd with the author first.

I think it depends on the context (specifically: hard news vs. opinion matters a lot)
