Must be something worse than the following definition. Do we know what "(S)" stands for? A violation of a control system?
Kind of amused by this glossary to a Department of Defense report, where the terms are not classified but some of the definitions are.

the (S) means "this definition is classified secret" — some of them are real head-scratchers.
