omg I finally found it. I e-mailed it to someone just last year, and Outlook had it in the sent mail (but somehow couldn't find it when I searched by it). weirdness: the filename is totally straightforward. yet I don't have the file anymore. that makes me... unnerved.
It seems that I just really don't have the original file anymore (I did a bash search for it). My theory is that I moved it into a Dropbox folder I use for sharing PDFs, didn't realize I had moved and not copied it, and then later deleted it. But I don't really know.
