I think we're seeing that an infectious outbreak will rapidly spiral at the point the hospitals hit capacity and you can't spin out enough beds to avoid sending sick people back into the community. Likely part of what's happening here, a function of demographics + infrastructure

Probably not, though cities that have erected massive regulatory barriers wrt land use/construction codes/permitting/liability (ahem, SF) will be in much worse shape than others
