A while back, I made it my business to figure out whether birth was preferable in a hospital (with aenesthesiologists) vs. in a birth center without pain relief. I found it difficult to draw hard conclusions, but even that makes me think the medical system needs to do better

The reality for a hospital birth is that the experience is likely considerably less personal, less communicative & more restrictive, with strangers probing you at your most vulnerable; in a birth center, you'll have more emotional support & freedom, but the pain is what it is

Most women who decide on birthing without pain relief focus heavy on alternatives - Hypno birthing, breathing exercises etc. I think it really comes down to their determination and mind set. Some believe link with epidural and more difficult (complicated) deliveries

This is selection bias on the types of people who choose birth centers. Natural birth is more a part of their identity.
