People often read this as a deliberate conspiracy, but part of the tragedy of it is its nature as a product of unanticipated consequences and silent costs of the “do something!” mentality among essentially absent and ignorant decision makers/voters.
Something Must Be Done, and it needn’t necessarily be effective — we don’t always even know what efficacy would look like, exactly — but it MUST be *visible*

More than that, the two feed on each other. Visible “reform” reassures us that all is well as we return to spending 95% of the time ignoring how the sausage is made, while that very neglect gives outlier stories their shocking power to ensure that Something Must Be Done (again).
