An addendum to this thread: there are good alternatives to call-out culture, e.g. dynamics built around the principles in @slatestarcodex's comment policy → honesty + social gentleness. I think that's so common because social capital is *the* thing that makes it so difficult to leave toxic cultures in the first place.

People think of all the relationships and status they'd have to give up if they left and started from scratch. Hoping things get better they stay, invest more in these things and the perceived cost of leaving only goes up.
