If you read the paper, the language he's advocating (OBJ) looks *a lot like* SML, except minus higher-order functions. I think that's the interesting part of the claim: if you took SML, removed HOF, you'd still be just fine.
I think about how modules in SML don't feel simple. A lot more syntax, and clunkiness compared to writing functions. Resurrecting a dead thread, but it seems to me that if what you have are values, functions, and modules, each parameterized over the previous, what you really have is precisely *second-order* programming.

zeroth order: basic data, booleans, integers, etc first order: functions second order: functions over functions third order: functions over functions over functions etc and 'higher order' is ambiguous between meaning "more than first order" or "all the levels" ("ω-th order")
