Well, there's OpenOffice, which after its corporate beginnings got dumped into Apache and then its real life-blood was transfused into LibreOffice and The Document Foundation.https://opensource.com/article/18/9/libreoffice-history …

Hm. Not exactly what I’m looking for, since that’s kinda your bog standard forking situation. And once it was bequeathed to ASF, OO.o has stayed there. My next project may include study of forks though, so it’s still info for me to track. :)

There are more cases of projects reaching their end of life and being archived than leaving.

Truth, but leaving is what interests me right now. It’s rare enough that knowing more about how/why could be useful for the community. Learn from the experiences of our forebears, etc.
