This seems like a weak standard. How many times have they tested the system?

seems to have a regular testing protocol.. that's how the accident happened. They sent out a real alert instead of a test alert.

The choices on the dropdown were "Test Send Missile Alert" and "Send Missile Alert." This is apparently bad design? Was the person operating this system confused about which one they should choose?

The visual appearance of the test a d real options were similar, the process of navigating to either of them was the same, and the process of sending a real alert was the same sending a test one. It looks like bad design.

It seems like a different system or interface design could have prevented this accident without impacting the other uses of the system, suggesting at least that better design is needed.

It should never be that easy to send a real alert when you mean to send a test alert. Nuclear reactor meltdowns have happened from similar mistakes - I’d say the acceptable threshold for error is zero
