Just came across this long-term quintiles of inflation-adjusted US household income - amazing the difference. If you didn't believe the wage/productivity divergence, it's hard to argue with this.

Ah the cost disease argument. It accounts for some things, like increasing income share of healthcare and education, but seems a bit too convenient

Agreed - I think when you view it by quartile growth it's more clear that it's top line income diverging between classes.
