Very curious about this phenomenon. I had a few episodes when younger, but not in a decade or more, mainly because I've largely taken myself out of that kind of stressful situation.

I think all of my final exams at Oxford were like that, except for one. In that one, I blanked in the exam – lost half an hour in a pure mindwash panic – and consequently I remember it everafter.

Yes! I order to perform a song, I keep forgetting bits of it until I can feel a specific state or voice for that song only. Then it comes naturally.

We used to talk about it in marching band all the time. Was more common than not. A big 15 minute collective blank spot from the moment we stepped on the field til the moment we left.
