People should use words like “incompetence” or “apathy” or “we’re stupid” in apologies like this. If you deny malice, you must accept stupidity. Otherwise it’s a get-out-of-jail-free card for unaccountable bureaucrats who were just spectators of miraculous errors without errants.

Similar experience. Running conferences has become fraught with all sorts of risks that simply don't make it worthwhile for the organizers. Esp if its non-profit and run by volunteers. Just not worth it. It used to be so different 10 years ago. The road to hell...

I feel like there’s some percentage of people who are uncomfortable with conflict, criticism, or negativity and believe they can prevent all anxieties by making rules against the situations that create them.

This is very different than preventing abuse and toxicity, but gets treated the same.

Yes, many of the CoC crowd have no sense of govenance/RuleOfLaw/etc so the odds of screwup are super high. I'm sure Martian Muskoprudence will be better.
