How does paid sick leave work with flexible shift work scheduling in service industries? The kind where you learn every week if and how much you’ll be working? Do such jobs guarantee hours/week to begin with?

You assume they coexist. I've yet to see them do so.
Service industries don't even do UNPAID sick leave. Failure to show to your shift can get you fired, no matter the reason. And no, there's also no guaranteed hours.
I'm sure that won't have any bad knock-on effects today. /s

I may be generalizing over all service industries based on the ones I worked when I was poor, and the ones my friends still work to this day.
But the ones you need to worry about today are the ones which exploit the poor. That's a huge infection vector with no solution in sight.
