If you are a publisher of any sort, how far out do you have content queued up to publish? In both time and content units (eg: 3 weeks, 6 blog posts)?
I'm nearly 100% just-in-time with no queue, except for my tip-of-the-day tweets which I batch 2 weeks ahead

I'm honestly amazed at people who have weeks worth of content canned and scheduled to publish automatically. I've occasionally managed 3-4 weeks out to clear the decks for a surge on another front, but it's horribly painful to do.

By queue do you mean planned or produced.
Planned ~3 months.
Produced ~1 day if I'm lucky and often late

produced... I don't take planning seriously :D

planned: 3+ months
produced: always completed three nanoseconds before deadline
Just-in-time! Farthest out I've seen is Tynan's yearly batching: tynan.com/tendays
