Planescape (it's a D&D setting) has this really fleshed out. Portals can be almost anything, materially speaking, but they all share important attributes. They require a specific key to unlock. They are mostly invisible to locals, but readily perceived by extra-planar travelers.

some portals are stable while others are naturally unstable, only being openable under certain conditions, or just randomly. there are also naturally occurring portals that don't require a key. these are chaotic vortices that are exceptionally hazardous.http://4e-planescape.wikia.com/wiki/Portals
