What about other things—When preparing a talk, do you practice it out loud? Do you think out loud?

Very rarely, and it tends to make for poor talks with me. This is why I hate doing short talks (TED length or lightning... 7-20 minutes) because the tight constraint forces you to rehearse far more than I like to. My best talks are 60-90 minutes.
