Wonder if organizations should be designed on the basis of the leak-before-break principle used in high pressure boilers/pipes
All orgs have internal reality distortion/false consciousness fields. The question is how/when containment fails, not if.

In a way, whistleblower policies that have become common in large companies are intended to do that. Whether they work well, or whether they are a survival-driven, preemptive and perfunctory move is a different matter
