Generally, with a glock (what most law enforcement uses) the safety is in the trigger. He prolly hit the trigger when grabbing the gun.

Yeah, what everyone else said, he looks like he pulled the trigger when he picked it up, he should have had a better holster, and you don't grab a gun that just fell, it's safety systems will keep it from firing (almost any modern gun), pick it up when you can do so safely.

Also I’ve seen law enforcement walking around with round chambered & hammer cocked back. While this allows you to fire quickly it also increases odds of stuff like this. Always seemed like a bad idea to me.

Glocks don’t have safeties as you’re thinking of them, and it appears he put his finger right into the trigger guard to pick it up. Guns don’t just go off, idiots pull triggers.
