      Let's get started. Scientists in the UK have warned that the variant first detected there - B.1.1.7 - may be associated with a higher degree of mortality. What specifically about that strain makes it more deadly?

      The main datapoint that I'm aware of here is the evidence of higher viral load in B.1.1.7 infections (https://www.medrxiv.org/content/10.1101/2020.12.24.20248834v1 …). We've seen elsewhere that viral load is associated with severity, so it makes sense to me that B.1.1.7 could be slightly more severe based on this.

      You don’t sound convinced....@EliLilly CEO David Ricks told me on #FacingForward that the US isn’t doing enough surveillance to adequately detect new virus strains & doing more is necessary to keep the #COVID19 vaccine & treatments updated. How would you describe US preparedness?

      These metrics can be parsed in different ways. If we just look at raw counts of sequenced viruses it's the UK, the US and Denmark in that order. However, the statistic I pay the most attention to is number of sequences available from samples collected in the past 30 days. 1/2

      Here, the US has gone from an average of ~100 sequences available from samples collected in the previous 30 days during the fall to over 5000 at the moment. We could be doing more, but labs across the country have really stepped up genomic surveillance measures. 2/2

      The pace of evolution is jointly determined by the population size of the virus (how many infections are occurring) alongside immune pressure (with intermediate levels of immune pressure driving evolution the fastest). 1/2

      Because the prevalence of the virus is so important to this equation, I'm convinced that vaccinating heavily will be much better than letting the virus continue to spread to curb evolution. Vaccination creates immune pressure but doesn't create onward infections. 2/2
