Borrowed from Malthusian-Ricardian economics. Who ever thought that we would see indices of well-being, such as life expectancy, decline in absolute terms in a developed democratic nation-state. We buried Malthus too early.

There's no physical shortage of food or medicine these days, so I'm not sure classical Malthus is what's at work here. Class imbalances as you mention seem to be the biggest factor here.
