Cold hurts more. You only need to use cold and compression to stop the swelling, otherwise heat helps most people more as it makes the muscles relax and release. I hate using cold even for the swelling, it always hurts me really badly.

Ditto on elevation - elevation is just to get the liquid to run back into your body. You should rest it as comfortably as possible for your own comfort, but it doesn't necessarily need to be elevated - that can put more strain on your muscles as they heal.
