I think it is mostly self-identification, at least originally. But of course then it gets used as an insult.

There are incel reddit groups, which shows its self created. Years ago these communities self identified as "forced loneliness" etc, and kinda evolved or devolved depending on your perspective. It kinda lends itself as an insult naturally though
