I suspect the costs for the average Covid patient who need ICU care is higher than the average ER patient in ICU if there is any room for them still in many of those poorly managed red states in terms of Covid.

Many countries with public Healthcare do impose sin taxes on things that are bad for you eg:cigarettes, sugary beverages, etc. Moreover, surgery and appointment wait times are often very long. Hard to point to a perfect health system and the money does need to come from somewhere
