I'm 100% sure jailer (aka helm) was in control. Arthas's mind defeated Ner'zhul prior to sitting on the throne but because his soul was already stolen/fragmented, he could not hold on against the helm's control for as long as Bolvar did. He's freed when the helm comes off

They bring Ner'zhul back (lore wise) as a presence in the helm (or technically Frostmourne) during the blades of the fallen prince legion artifact quest. He's still fighting to control Arthas
