@siracusa I think it may have been fixed in 10.8.4 or 10.8.5 - just realized I haven’t fired off my fix-the-icons script in weeks.
This bug is saying its final goodbyes to me these past few weeks: http://cl.ly/image/2z2J240m3A0W … (I haven't seen it in Mavericks…fingers crossed!)

@siracusa it was happening to me too, but stopped after a restart. Has not reappeared again. I think one of the ML updates fixed it.
